I am not sure of all the circumstances that this occurs.

Opening an HTML Link (default browser = Firefox 3.5) results in a hidden window [window appears in the task bar, but cannot be made visible or active].).

I have sent my config file to Support.

Dear Terry,

Thank you that info us about the problem. It was confirmed.

We'll fix it. But now as workaround we can advise you to create the rule for Firefox with following parameters on Target Panel (class: MozillaUIWindowClass, program: \firefox.exe and disabling Window Caption). Or change your Mozilla Firefox rule - switch radio button on Target Pane to Substring match value.

Tatiana,

I will keep those workarounds you offer in mind.

However, currently the simplest workaround I discovered is to uncheck the
Code
"Block Premature Appearance Of Window"
option.

...Terry.
