My environment:
Win 7 64-bit on a Dell E6420.
Actual Multiple Monitors version 4.0
Using laptop display (primary monitor), Dell E193FP (monitor 2), and Dell P2411H (monitor 3). The monitor layout is 2, 3, 1. Monitor 1 is set to be the primary. Monitors 2 and 3 only have "Extend my Windows desktop onto this monitor" checked.
When screen saver is set to "Individual screen saver on each monitor" and using a the Screen saver "Photos" (cycle speed set to Fast), the first picture in the folder pops on all 3 displays but it does not cycle through any of the other photos in the folder.
When set to Single screen saver over the entire desktop, the picture appears on monitor 3 but does not cycle through the photos.
When set to "Current Windows screen saver", the photo pops on monitor 2 with some overlap on to monitor 3, but the photos in the same folder will cycle.

This bug has been fixed, check it out in the
latest 4.1 beta
.

Tried installing the 4.1 beta, however it hung my explorer.exe process as soon as I clicked on the start menu. Had to reboot in safe mode in order to get it off the system as I don't have time to debug this, or to test if the beta version fixes my picture cycling.
When do you expect to publish 4.1?

4.1 is about to be published in the near future, maybe next week.
