I upgraded to Title Buttons v4.2 earlier today from a 3.x version primarily to take advantage of the Grouping Button.
Although I've attempted to enable it a number of times, it never 'sticks'or shows up. I also noticed that when I click on the check box to enable it, the 'save' button doesn't change states. This indicates to me that it isn't seeing the control.
I've also notices some unexpected behavior with multi-monitor. I'll report more when I have more info.
Thank you for your attention to this matter.

John,
Thanks for your notes! The "grouping button" issue was fixed so please re-download the
ATB installation package
and re-install ATB.
Also waiting for your report about the multi-monitor issues.
