"Maximize to desktop" and "Move to monitor 2" appear directly to the left of my Windows "Minimize" title bar command. Can I delete these 2 buttons? If so, how is this accomplished?

Hello Rob,
They can be deleted here:
Actual Multiple Monitors -> Window Settings -> Default settings (or Specific settings) -> Title Buttons.

I am having this same problem. Win7 64 bit. I have verified that they are supposed to be off, but unfortunately this feature does not seem to be working.

It does not appear that all applications have this problem. Some notables, Microsoft Outlook 2010, latest version of Google Chrome, latest version of Skype, Microsoft Excel 2010.

Hello, Damon!
All the applications you specified have their individual rules enabled in Specific Settings of Actual Multiple Monitors by default.
You can remove title buttons individually for each of those applications here:
Actual Multiple Monitors -> Window Settings -> Specific settings -> Google Chrome (or Skype, or Microsoft Excel, etc.) -> Title Buttons.
