The title bar buttons appear fine when Firefox (I'm on v 26) starts up, but if I click a link that opens a second window, they do not. See the attached screen shot for an example. I'm sure it's a setting somewhere but I've not been able to find how to fix it. Any help would be appreciated.

Good day,
You are right,it is a "setting trouble".
When you click the link that opens a new window it can have a different window class so rule is not applied to it.To fix this, just go to the Special Rules section(or Default Rules,if you the rule for FF is there) and uncheck window class criteria and window caption criterias.
